Create Your Lead Form in Chatavise
Before installing a form on your website, you must first create it in Chatavise.
Log into Chatavise and navigate to Marketing > Lead Forms.
Click Add New Form.
Enter a name for your form.
Select the business profile (location) the form should belong to.
Configure the form fields, opt-in messaging, and styling.
Save the form.
Once saved, Chatavise will generate an embed code and a standalone link for the form.
Option 1: Install the Form on a WordPress Website
If your website is built on WordPress, follow these steps:
Open your WordPress dashboard.
Edit the page where you want the form to appear.
Add a Custom HTML block (or use your page builder’s HTML widget).
Paste the Chatavise form embed code into the block.
Save and publish the page.
Your Chatavise lead form will now display directly on your WordPress site.

Option 2: Install the Form on Other Website Builders
For website builders such as Wix, Squarespace, Webflow, Shopify, or custom HTML sites:
Copy the form embed code from Chatavise.
Open your website editor.
Add an HTML Embed, Code Block, or Custom Code section.
Paste the Chatavise embed code into that section.
Save and publish your changes.
Most website builders support custom HTML blocks, making installation quick and flexible.

Option 3: Use a Standalone Form Link (Optional)
If embedding a form isn’t ideal, Chatavise also provides a shareable standalone link for every lead form.
